Fetal Echocardiography (Fetal Echo)

Fetal Echo is performed by Dr Samta Parteki, MD (Radiodiagnosis) β€” a detailed ultrasound study of the fetal heart to detect structural cardiac defects before birth.

Fetal echocardiography is a specialised ultrasound examination of the developing baby's heart. While a routine anomaly scan includes a basic 4-chamber heart view, fetal echo goes much deeper β€” evaluating all cardiac structures, valves, great vessels, and flow patterns using Colour Doppler and pulsed wave Doppler.

Fetal echo is ideally performed at 22–24 weeks of pregnancy when cardiac structures are well-defined and the baby is large enough for detailed visualisation.

When is Fetal Echo Recommended?

  • Family history of congenital heart disease (CHD) in a parent or previous child
  • Abnormal cardiac findings on routine anomaly scan (4-chamber view, outflow tracts)
  • Maternal diabetes (pre-gestational or gestational diabetes increases CHD risk)
  • Maternal autoimmune disease (lupus, anti-Ro/La antibodies β€” risk of fetal heart block)
  • Maternal infections β€” rubella, CMV in early pregnancy

  • Abdominal ultrasound: Fast 6–8 hours (no food). Water allowed.
  • Pelvic ultrasound: Full bladder β€” drink 4–6 glasses of water 1 hour before. Do not urinate.
  • Obstetric scans (1st trimester): Full bladder preferred. TVS available if needed.
  • Obstetric scans (2nd/3rd trimester): No special preparation.
  • Fetal Echo: No preparation needed. Best at 22–24 weeks.
  • Thyroid, breast, scrotal: No preparation. Wear loose, accessible clothing.
  • Doppler studies (vascular): No preparation. Avoid tight clothing over the area to be examined.
  • Renal Doppler: Drink plenty of water beforehand for good kidney visualisation.

Is Fetal Echo the same as anomaly scan?
No. The anomaly scan (Level 2) includes a basic 4-chamber heart view, but Fetal Echo is a dedicated, detailed cardiac examination lasting 30–45 minutes. It evaluates outflow tracts, valves, vessels and flow using Colour Doppler β€” detecting defects missed on routine scans.
When should I get a Fetal Echo?
Ideally at 22–24 weeks of pregnancy. It can sometimes be done earlier (18–20 weeks) if needed. Referral is recommended if you have a family history of CHD, maternal diabetes, or abnormal NT scan.
